Notes

In common usage a Defect and a fault are not distinguished from each other. Here we want to use the terms so that “Specification-Defect” refers to a specification problem, and fault refers to a real artifact problem with actual potential for a Malfunction to occur as a Result.

2. Further, we are sharply limiting the notion of “Specification-Defect” to a violation of some agreed and defined Standard for specification (a Specification-Rule), not just one person”s personal opinion about “style”.
