Definition:

A System has a Malfunction when it does something not intended, or not desired, according to its design/construction specification, or according to reasonable Stakeholder expectations (even though there may be no official specification about that expectation). Malfunctions are undesired outputs (including no outputs) from a “real” System.

1. Severity: a Malfunction can have any Level of severity or Impact from catastrophically permanent destruction, to hardly noticeable variability of information or timing. It all Depends-On the nature of the specification, which is violated. Consequently, to really understand a Malfunction, a notion of severity of Impact needs to be added. This Impact all Depends-On the environment impacted at the time of Malfunction.

2. Human System Stakeholders, users or operators may commit errors in connection with real Systems (as opposed to specifications of those Systems), which lead to System Malfunction.
