Examples

A geological fault is a Defect in the earth. If the fault is provoked, the Result can be an earthquake (a Malfunction).


Notes

1. “Bug” is a common synonym for “Fault”.

2. Figure *339 tracks the chain of events leading to a Malfunction: a human engineer makes an Error. This may be detected as a Specification-Issue, which in turn, may be classified as a Specification-Defect (which then may, or may not, be fixed). Alternatively, maybe the problem is not detected as an Issue, and the Error simply becomes a Specification-Defect. A Specification-Defect is in the System specification used to implement the System, it may become an actual fault in the implemented System. If the fault is then hit (provoked), a Malfunction of some kind may well Result.
